Request
Under the Freedom of Information (Scotland) Act 2002, I request the following recorded information for the period 1 April 2024 to the date of this request:
- Copies of all reports made to your board and any of its committees by individuals with lead clinical professional responsibility under the boards procedure for reporting on compliance with its duties under the Health and Care (Staffing) (Scotland) Act 2019.
- Please include every quarterly report in this period. If any quarter has no report, please say so.
- Any entry on the corporate/divisional risk register that refers to unfilled shifts/the inability to staff wards safely.
- Please include the risk description, risk rating, date raised, and any record of changes in rating/or of the risk being closed or downgraded.
- The number of times the boards real-time staffing and risk escalation procedures were used, by month, and for each occasion: the hospital site, outcome, and whether the risk was recorded as resolved.
- If not recorded centrally, please state this and where it is recorded instead.
- The number of incident reports (Datix/equivalent) wherein inadequate staffing/unfilled shifts were recorded as a contributory factor, by month and by severity category.
- Any report/paper/briefing from your board or its committees that analyses unfilled shifts and/or their effect on patient care. a. Please provide the papers themselves, and not summaries.
- Any correspondence between the board and the Scottish Government, or, Healthcare Improvement Scotland, wherein unfilled shifts/staffing shortalls were raised as a concern.”
