- “Your report states that of the 4,804 individuals discharged from
hospital to care settings between 1 March and 31 May, that 675 of them sadly died within 30 days of discharge. I request that the following additional information be provided:
- i) The number of discharged patients that died within 60 days
- ii) The number of discharged patients that died within 90 days
iii) Of the 675 patients that died within 30 days. Please provide the number that died whilst in transfer (ambulance or similar) between Hospital and Care Home setting.
iv) Of the 675 patients that died within 30 days. Please provide the number that died within 7 days of transfer.
