This document contains all current guidance relating to OPCS 4.11 Clinical Coding in Scottish Morbidity Records (SMRs).
All discharges from 1st April 2026 should adhere to the guidance given in this document and subsequent publications of the Scottish Clinical Coding Standards.
Titles of standards made obsolete since the last edition of the consolidation are listed at the end of the document. Obsolete standards are not included in this document, but are available on request from phs.terminologyhelp@phs.scot
