The Public Health Scotland sequencing service aims to sequence all samples which test positive for SARS-CoV-2 in NHS Scotland.

This will be used to inform national surveillance. This sequencing will be performed routinely for all samples which meet the laboratory acceptance criteria and does not require an additional request form.

In some situations, however, there may be a clinical or public health need to request additional bioinformatic analysis on an individual or groups, for example incident or outbreak investigation.

In these cases, this additional analysis request form should be completed.

Please note, this additional analysis is only possible on suitable samples which have already been sent for routine whole genome sequencing (WGS) surveillance. So, only samples with cycle threshold (Ct) values below 30 should be included in the request. Failing to do this could unfortunately cause delays in the process.

Please complete both sections of this form for any samples which require cluster or incident investigation.

  • date of birth 
  • lab specimen number
